Lombardi Cancer CenterFellowship Program In Palliative Care Medicine
Andrew Putnam, MD, Assistant Professor, Fellowship Program Director, Palliative Care Program

Dear Palliative Care Applicant:

Thank you for your expressed interest in the Palliative Medicine Fellowship Program at the Lombardi Cancer Center, Georgetown University Hospital. Below is detailed information about the program. You can download an application from this website and complete, and mail it with your supporting documents to us. Please make sure you indicate which year you wish to start your fellowship on the application form. You may also look over the Hematology/Oncology Fellowship Training Program because it highlights our institution and program in which the Palliative Care fellow participates.

In order to be considered for the Palliative Fellowship, please forward the following:

  1. One completed fellowship application.
  2. One copy of your curriculum vitae and your personal statement.
  3. Arrange for three letters of recommendation from instructors/physicians in your current and/or past training program to be sent under separate cover, not with your application. Arrange for one letter of recommendation from a research mentor if you have done research.
  4. Copies of pertinent documents (i.e., medical school transcript, diploma, evaluations, publications, certifications-ECFMG certificate, and any unrestricted license if applicable).

After we review these materials, we may contact you to arrange a personal interview at Georgetown with the Faculty of the Division of Hematology/Oncology. Please provide a daytime telephone number, e-mail address and/or fax number where you can be reached.

We look forward to hearing from you, and again, thank you for your interest. If you have any questions please contact Adrian Hameed, Fellowship Program Coordinator, Hematology/Oncology and Palliative Care Fellowship Programs at (202) 444-7036 or e-mail at ah@gunet.georgetown.edu.

Sincerely,

Dr. Andrew Putnam
