Virginia License Paperwork
Below you will find paperwork for your Virginia training license. Please choose the appropriate document based on your training track.

DEADLINE = FRIDAY, APRIL 15
Send your completed application, $55 check, and passport-sized photo to our office by the above deadline. The mailing address is:

Once we have reviewed your application and ensure that no sections were missed, we will mail it, along with the Form A (see note below), directly to the Virginia Board of Medicine on your behalf.
The application has been pre-filed to include start & end dates along with the name of the affiliate sites where you will be rotating in Virginia.
Our office will complete Form A for you and send it along with the rest of your application materials, once we have received them from you.
Form B must be given to your medical school Dean’s office to complete and they must send Form B to the Virginia Board of Medicine directly. Please do not send Form B to our office with your application; processing of your license will be delayed if you do so.
VERY IMPORTANT: Your medical school CANNOT mail Form B until AFTER you graduate. If your school mails the form before you graduate, the Virginia Board will not process your application.
We recommend that you deliver Form B to your Dean’s Office in person the week before you graduate, and explain that the form needs to be sent the next business day after you graduate. Please attach a note to Form B for the person in the Dean’s Office who is actually preparing the paperwork explaining this.
In addition, you may wish to consider including a pre-addressed and stamped envelope for the Dean’s Office to use so they may simply slip the completed form in the envelope and throw it in the outgoing mail.
Please note that training licenses must be renewed every year. It is your responsibility to make sure this is taken care of in a timely manner.
